You fill in the form and accept the terms
Personal details, medical consent, touch-up policy, general terms — all online.
You confirm and submit the form
A summary is emailed to you. I receive a notification of your submission.
I complete my part and we both sign
I add the session details (ink, materials) and sign the document. You receive the final signed PDF.
